Looks like there are a couple issues going on that are causing this among other people. One other option you might try, and the way I solved my issue, is to throw the installer back on your server in the FT directory and run it. It will show you any issues with your setup before it gets through everything and before it touches the DB.

Yeah I guess that may be my only option. I have 5 of these forms on different sites though so will be fairly time-consuming! Guess I'll check back tomorrow and if it's still not working properly I may have to resort to as you suggested above! Thanks again for your reply! ;-)

It sucks maaan! lol I've done 4 out of 5 forms so far this morning by individually uploading the 'install' folder then creating everything from scratch again as on the original install. AND.. 2 of my 4 forms didn't even work by doing that so I had to delete the entire formtools folders from my server, re-upload then continue from scratch that way! Really annoying seeing as I have no idea why it was working on Wednesday and not come Sunday! I JUST HOPE FOR MY LIFE THIS NEVER HAPPENS AGAIN! :/ It's taken me the best part of half a day so far! :/
