How can I prevent duplicate entries - azism - Apr 26th, 2016

I am trying to create a form to be used to vote from a selection of several candidates who will be selected for their achievement(s). How can I set up the form to prevent duplicate votes based on IP address to prevent "stuffing the ballot box"?
