Could you try running the test in a different browser (FF/Safari/Chrome) and let me know if you get the same error. The Database Integrity module is no longer supported - I'm going to update the compatibility right now to only have it go up to Form Tools 2.1.4. For Form Tools Core 2.1.5 onwards, just use the System Check module Thanks for letting me know! - Ben
