Beginner: how to link a field to an existing field in a MYSQL Database - ACGL - Jan 6th, 2017

Hi,
I am a beginner with FormTools. How can you link a new form to an existing Mysql data base on my webserver.
The FormTools is installed on my Webserver too.
I have not found any documentation which describes how to link the fields of the form with the fields in the database.
Please inform.

I am having troubles with the form builders do you guys have the sql files needed? for me to manually add the missing lines?
