Hi,

I would like to know if it's possible to add a logo or image into the header of my form? Does Form Tools have this capability?

Most likely! Are you talking about an internal form, using form builder, or just a form you built?

If you are trying to add a logo into a form built with the Form Builder you will need to revise your template set: http://modules.formtools.org/form_builder/?page=template_sets

I am not a big fan of inserting images into forms. I think it makes your website look cluttered and unprofessional. I personally recommend https://masterbundles.com/templates/presentations/powerpoint/animal/ source to download templates. The good thing about it will be easier for them if they can see your logo or image while filling in their information.
