MySQL Error - NorthernPenguin - Aug 30th, 2015

I am using v2.2.6 of Form Tools. I created a data visualization and can see it properly within formtools. However, when I copy the php code and place it in a file on my site, I get the following error:

MySQL Error: Table 'rsekaly_formtool.npt__error_log' doesn't exist - in errorlog_functions.php on line 200 by mysql_insert()
I checked with phpMyAdmin and there is no table of that name. There is an rsekaly_npt.npt__error_log.
