Add Form - Step 5 Problems Chasing my tail - Iwritetheforms - Sep 21st, 2015 I'm having difficulty getting past step 5. I've done another form on one of my other websites with no problems but for some unknown reason I'm having difficulty on 2 other websites. I'm hosted with a Godaddy cpanel server. I have swift mailer enabled. Error message: There were multiple fields that couldn't be found in the form page you specified.
