Submission Accounts add Form Name to Edit Screen - timhoth - Oct 1st, 2015

Using Submission Accounts, when I edit a submission I want to know which form I am editing. How can I change "Edit Submission" on the top of the edit screen to "Edit Submission - form.name" so when I have multiple similar looking forms I will know which one I am editing?
