Hooks Manager and Extend Client Field

Hi guys, I'm new to PHP coding and Form Tools and I'm having a hard time getting the three data from the Extend Client Fields to the form. My reference was:

I have three extend client fields namely:

Area
Department
Section

I want these three to automatically go into the form upon creation.
