Client information in form?

Hi,
is it possible to add the client data to the forms database?

I am developing a feedback system similar to this:
lets say I have 10 teachers (my clients), 15 students and 4 years to provide feedback for.
Now each teacher should be able to fill in a form for each student each year.

I can check if a student feedback has been received for the relevant year, but I need to also know which client has submitted the form.

How do I do this?
Can I have a hidden client field?
