I can't seem to find what is causing this:

When I logout of a user submission account, this URL comes up with the wrong ID.

.../submission_accounts/login.php?form_tools_form_id=4

It should be:

.../submission_accounts/login.php?form_id=4

I have this URL in settings:

...my_site/FormTools/modules/submission_accounts/login.php
