Trouble With sending the test Sublimation

Hi there,

i have used HTML to create the from. And have used form tools for two other forms i have. Now when trying to submit this form, it will not send. I have included the form action and input type="hidden" name="form_tools_initialize_form" value="1" &
input type="hidden" name="form_tools_form_id" value="5"

But yet is still not sending the test.
Any help?

did you follow the instructions on http://docs.formtools.org/tutorials/adding_post_form/?
what happens when pressing on your submit button? nothing at all?
could you post your code in here?
