I'm getting a 500 - Internal Server error at step 5.

Not able to get the admin account to be added. Everything worked fine up to that point. Any suggestions on what I should check next would be great.
Quick update, I checked mysql and the account table and the admin account has been created. The server 500 error is actually occurring at step 6. So it is getting hung up on the clean up step.
