It looks like you chose to create an internal form. Check out the documentation here for some more explanation about the difference between internal and external forms: http://docs.formtools.org/userdoc2_1/?page=form_types
You will either need to use the form builder module or create an HTML form on your own that will connect to what you created in the formtools backend. Give it a try and post if you run into problems!
