My Submission Accounts isn't working. When users try to login to manage the form they can see the proper menu along the left but along the right it just shows the login window again. I have checked all of the settings and copy and pasted the code from the formtools submission accounts custom menu.

I only go to this page if the username and password is correct. Otherwise it reloads the login page.

Has anyone else had this issue?
