somewhere I see : don't start from scratch but take a default form and make your changes.
but after making any form, how and where can I find the php-code?
or any php-code as an example?

Are you saying you have an existing form you want to connect to FormTools? Or what php code are you looking for?

I wonder if you could add the Form Builder module to FormTools, create your form with it, and then view/modify/edit the code from the resulting files. I'm not certain if the PHP code would all be present within the files generated by the Form Builder module, or if it is still called as needed from the many PHP files included with the module, but it is something for you to try.
