error after upgrading from 2.02 to 2.1.5

I upgrade form tools from 2.0.2 to 2.1.5

when i submit to my form

Sorry you're having problems!

First off, check you're running a compatible version of the API. Since you just upgraded, you should be running 1.1.4:
