Hi,
You'd normally create a MySQL database via your web hosting account (Cpanel or similar). Once that database has been created together with a user that has access to that database, you can then install FormTools.
FormTools will do the rest of the work for you!
If you get stuck just google some of the points above, there are lots of tutorials out there.
