Hi Mattayo,

Well, your form looks okay.

Can you upload files through the Form Tools interface? To check, just edit an existing submission and upload a file. If that doesn't work, it should give you a descriptive error message about why it failed (I think.... my memory is failing me in my old age...).

Let me know how that goes.
