It might be that you have selected the wrong field type for the respective fields during the setup process. Here is how you can check and change that:
- Go to “Edit Form”
- Click the “Fields” tab
- Look for your file upload field on the list
- Check the “Field Type” column, “File” should be selected in the dropdown
- Change if not
- Go to “Settings”
- Go to the “Files” tab
- Click “Test Folder Permissions”
- If you get an error message you will have to change folder permissions
- Check the file size limit and whether you can actually upload the types of files you tried to upload (otherwise the upload will just quietly fail)
