I'm not yet familiar with the Form Builder, but I would expect, as a module, that you need to login to the admin portal, then click on "Modules" from the left-hand navigation.

If you've properly installed the module already, then you should see it in this list. It may have an option over to the right that says "Upgrade." Basically, you're just telling the Admin Portal to go ahead and use this new module.

Then again, I could be WAY off base, since I've never used it...lol. But give that a try?
