If you just made a form directly within FormTools you ended up with what is called an internal form. This form is just available from within FormTools and you need to have an account to reach it (you, as an admin, have access of course). A typical use of an internal form would be to keep a personal register of some kind (or within an organisation with user accounts).
All your forms are found under the main navigation link (to the left) that says "Forms". There you will find your form (probably with the label "Internal" to the right of its name).

Form Tools is a tool to connect a form with a database and handle the submitted data, more or less. It's really not a tool to MAKE web forms (though there is a premium module for that).
FormTools help you set up the database environment, which fields you want, what type of fields and to administer the submissions afterwards. BUT, you build your own web page with the form fields on it (unless you make an internal form).
