For anyone else having similar problems - I fixed it by deleting formtools from the server and starting with a fresh download of the program. I previously uploaded it with Dreamweaver which timed out during the upload and I had to reconnect to finish it. This time I used filezilla and it went smoothly. Then I was able to go through the setup.

However, the "redirect" is not working. I checked the url of the redirect and that is correct.
