You certainly don't have to have the Form Builder Module, but if you want to make external forms without needing to know any HTML, yes you will need it. It makes it super easy.

1) Yes, you can set an email template to send an email to the person who submitted the form and/or anyone else you choose. You can create the template to include all of the data from the form. See the documentation here: http://docs.formtools.org/userdoc2_1/?page=emails
2) Yep, you can have as many recipients to the above as you want.
3) Yes, both the submitting user and any recipients of your choice can get a copy.
