Did you ever find a solution for this problem? I am currently having the same exact problem.

Form submissions do get entered into the database but notification emails do not go out. I tried deleting and creating a new email template for the form but even when I try to test the email in the editor I get the failure you are describing above. This happens both when I try to send a test email message and also when I just try the "Display Email" button in the editor.

I just get a message that states "Couldn't load page: http://mydomain.com/form-tools/global/co..._email&..." etc etc.

I've confirmed that the "actions.php" file is on the server and these notifications were working perfectly for a long time on this server.

Any insights?
