Hmm...try changing the name of the form id variable to $pp['form_id']:
Going by the tutorial listed on the site, it's defined in your library.php includes file if I remember correctly.
Unless you have the form id declared in one of the form fields you're submitting, there's no way for FormTools to know which form to retrieve the data from on your return page.
Are the entries being submitted successfully into Formtools when you submit the form?
