Stuck on step4.php

Hi,

I was using an older version of formtools which worked fine, till i mistakenly overwrote it.. so i cleaned up and installed the latest beta version.

I am trying to use formtools for my form now and it just won't work... I'm not sure what I'm doing wrong..

initially I thought that it was the php code at the start to get the required file linked.. so i modified it to the following:

$submission_hash['form_tools_initialize_form'] = 1;
$submission_hash['form_tools_form_id'] = 1;

?>

however it won't work...

I get stuck on step4.php where even though I try to enter the names or use smart fill it will end up being blank and just stay on that page.. I even tried to go to step5.php manually and upload the form.. it still won't work.. It seems to end up showing me half a page when I click on next step (on step5.php) saying some SQL error exists..

The same form was working earlier with the older version.. so I'm not sure what we can do to get it to work with the newer version..

I am actually running late in opening registrations for my users and hence sincerely request you to please look into the matter urgently.

If required I can also setup an FTP login and give you access to formtools on my website to look into it..

Hi Ben,

I've gone through your website and tried to implement Adding a single page form with the API and again get stuck on step4.php

this is wht the php header now looks like:

And when I try to go to step5.php and skip I get the following error:

Error:
Failed query in ft_finalize_form
