Hello
For some reason that I do not understand, I am unable to really get into Phase 3 of the Installation process. The page comes up but then all I get is:
"Now we're going to create the Form Tools database tables. This step requires the database itself to have already been created. If you're not sure how to do this, please contact your hosting provider. "
I do not get the forms below this where, I assume, I need to refer to where the MySQL database is located.

Is there a "next" option? I think the first page on phase 3 is just a notice to let you know what you're about to do. Click next to proceed, where I'm sure the install will ask you for that info.

Thanks for your reply. Unfortunately not that simple.
Just in case I was a right idiot, I zoomed out in IE to check that I wasn't missing something.
All I see on the screen is:
Menu on LH where 3> Create Database Tables is highlighted.

What may be of interest is that the links at the bottom which are present on Phase 1 and Phase 2 (formtools.org.....Installation Help.....Documentation..... Problems / Questions? Visit our forums) disappear at Phase 3.

Sorry, didn't mean to imply anything, just pointing out the obvious. I haven't done an install in awhile and I'm afraid I don't have time for a test install this week. But, honestly, I would try to start over. Remove your FT install files from the server, download a fresh copy, FTP them up to the server and try again. Sometimes things break during that process that cause strange issues like this. If you follow these steps and try again only to get the same problem, report back and let us know.
