Internal forms will require each user to log in to FormTools. If you want a form on your website for visitors to submit, you'll need an external form. Follow this tutorial:

http://docs.formtools.org/userdoc/?page=adding_forms

There is also a form builder module if you are less experienced with HTML.

You can make a new form and link it to formtools to process the form.

If you use an internal form you will need to have a user account for everyone who needs to submit the form.

Form Tools is an open source (free!) PHP / MySQL script that provides any existing web form with a backend database and a user-friendly interface to manage the form submission data, plus a wealth of tools for managing and manipulating your form data.

I'm not sure what your goal is, but if you need help creating the external form, there is a form builder module that will help you do that.
