Can't load Hooks Manager module

Long time user of Form tools here.
Been using it for several small web sites and the forum has been a great help whenever I ran into different issues. Finally need some specific help now.

I installed the Hooks Manager module (v1.1.3) and whenever I try to select and load it, I get the following error message:

ERROR
You have an error in your SQL syntax; check the manual that corresponds to your MySQL server version for the right syntax to use near '' at line 5

   

Can someone pinpoint where I'd correct this issue? I'm not all too familiar with PHP so I'm lost on where to begin looking!

I am a new user (just start using it today). I ran into the same problem now. Have you found the solution to share? Thank you very much in advance.
