An error in your SQL syntax

Hello there, first I'd like to say that I'm so thankful for FormTools, it's been great!

I've been running a html form for a long time with FT and I just recently duplicated the form and changed it a bit for a slightly different purpose...still very similar.

When I try to add it to FT I get the following error:
