Hi.
Have installed Form Tools on 1and1 virtual server using Plesk.
Chose installation path https://domain-name/formtools.

Trying to Add form.
http://www.domain-name/info/test/sample.html
This url was successful in Form Tools demo site

At this point Add form, Step 2, Verify URL produces a blank screen and does consider url as valid.

Any suggestions welcome.

Carrying on from above, the practical answer seems to be to click on Accept button, and carry on to Next Step.
