I created a form with Form Builder and published the PHP file. Then to match the form to the looks of my website I wanted to give it the default navigation (header and footer) on my website. Navigation does show up but the link in navigation are not working and following PHP errors are thrown as well (I'm using mmenu).

I want to know if this is the correct method of trying to apply navigation or should I go for a custom template?
If I've to have a custom template then please guide me how to do that.
