Add Form - Step 5 Problems Chasing my tail

I'm having difficulty getting past step 5. I've done another form on one of my other websites with no problems but for some unknown reason I'm having difficulty on 2 other websites. I'm hosted with a Godaddy cpanel server. I have swift mailer enabled.
