Hi Ben,

When you say the test works fine, do you mean the "Send Test Email" button on the Edit Form -> Email Templates -> Edit Email Template page? If so, that's a good start - at least we know the email functionality is configured on your server.

Check the first tab of the email template. That specifies when the emails should be sent. Is "on submission" checked?

Also, is this an API or POST form?
