It's easier than all that. The instructions are saying that if you uploaded the formtools folder to the root of your site, then you should be able to visit http://www. yoursite .com/formtools/ in a browser. If you go to that URL and you get an error, then there is a problem. But going to that URL (using your own domain name, of course), should open up a page that walks you through the installation process.
