Hi Tim,

Through the admin interface please go to Modules and then go to the bottom of the interface and click "Refresh Module List". Sometimes the modules do not appear automatically. If you it still does not appear you may need to delete the module and upload to the directory again.

If that fails, there may be some issues with the Form Tools installation and you will need to run a system check to repair any issues.

Please let me know if you are able to see the Form Builder module after these steps.
